REQUEST COMBINATION CODE AND/OR DUPLICATE KEY
STEP 1 - PREPARE REQUIREMENTS. To enable us to help you with the
combination code or a duplicate key must provide the items listed:
1.Completed Master Code or Key Request Form
2.Copy of valid Government issued ID
3.Proof of Purchase and Ownership
a. For Private buyers, sole proprietors:
- Proof of Purchase (e,g. Official Receipt/Sales Inv./Delivery Receipt) OR
- Notarized Notarized Affidavit of Purchase and Ownership
b. For Corporations/Partnership:
- Secretary's Certificate OR
- Notarized Authorization Letter by Officer of the company (as listed in the GIS) with an
attached copy of the company GIS
4. Payment for code retrieval / key
replacement
Step 2 - SUBMIT all completed forms to us via one of the following
methods:
